Home Health Care and Adult Day Health Care

Often, as people age, it becomes increasingly difficult for them to manage a household and their own health care without help. Our Home Health Care and Adult Day Health Care programs assist elderly veterans who live in their own homes. By providing quality care at home and at licensed facilities, we hope to keep veterans living independently as long as possible.

Homemaker Home Health Care

Homemaker Home Health Care provides Veterans with a Medicare state licensed aide for one to five days a week in the home.

This same service provides Respite Care to all eligible veterans and their caregivers. Each entitled veteran is granted 30 days per year of in-home Respite Care, providing relief to the primary caregiver for up to six hours on each of those days. In-home Respite Care is provided in addition to a veterans' regularly scheduled Homemaker and Home Health Care visits.

Our Homemaker Home Health Care program offers the following services:

  • Bathing, Shaving, Dressing
  • Ambulating
  • Medication Supervision
  • Laundry
  • Limited Shopping
  • Light Housekeeping

Skilled Home Care

Skilled Home Care provides Veterans with services from a licensed nurse, as well as, short term physical, occupational and speech therapy. A VA primary care provider must recommend Skilled Home Care in order for a Veteran to receive it.

Adult Day Health Care

Adult Day Health Care is daytime care provided in state licensed, community based facilities. Veterans receiving this care live in their own home and attend one to three days per week, from Monday through Saturday.

Our Adult Day Health Care program offers the following services:

  • Therapeutic Activities
  • Medication Monitoring
  • Personal Care (if needed)
  • Balanced Meals in a Supervised Setting
