Request Your Medical Records

You can access, obtain copies and request amendments to your medical records through the Release of Information Office, located in Building 4, Room 214C. They may also be reached at 610-384-7711, ext. 5069.

To request a medical record, send the applicable forms (VA Form 10-5345, Request For and Authorization to Release Medical Records or VA Form 10-5345a, Individual's Request for a Copy of Their Own Health Information) to the following address:

The Release of Information Office must receive original, signed forms and they cannot accept forms via email. Requests for records may take approximately 10-14 days to process. There is no cost to send copies directly to a health care provider. If copies are for personal use, photocopying fees may be assessed.

You can also access your medical records through MyHealtheVet, an online health management tool. More information about MyHealthe can be found under Health Education.

Virtual Health Information Exchange (VHIE)

VHIE Sharing Options

In accordance with the VA MISSION Act of 2018, VA is changing the rules for electronic health information sharing to give your community providers a more complete view of your health. Starting April 17, 2020, VA will seamlessly and securely share your VA health information with community providers who are a part of your care team. VA only shares your health information with community providers via VHIE when they are treating you.

You do not need to take any action for VA to start coordinating your care more seamlessly so that you have a better, safer experience. If you do not want your care team to receive your information via VHIE, you may opt out of electronic sharing.

How do I join?

Signing up is fast and easy. You may sign up in person, through the mail, by fax, or online.

  • Online: Simply visit VHIE online for a complete list of instructions on how to sign up online.

How do I opt out?

If you would prefer that VA not share your health information electronically, you must complete and submit VA Form 10-10164 to your facility's Release of Information Office (ROI). You may also submit this form online via My HealtheVet.

Please Note: If you have not already done so, you will need to upgrade your My HealtheVet account to Premium status in order to securely submit these forms. Visit My HealtheVet to learn more.

There is no deadline to opt out and you may do so at any time. Choosing to opt out will not affect your access to care from community providers. However, if you opt out, your community providers may not receive your medical records before you receive treatment. This may put you at risk. Additionally, if you visit any emergency room, your information may still be shared via VHIE so that you can receive the care you need.

Can I opt back in?

If you previously opted out but want to resume secure, seamless sharing, you may do so at any time by completing VA Form 10-10163 (also available in My HealtheVet).
