Request Your Medical Records

You can access, obtain copies and request amendments to your medical records through the Release of Information Office, located in Building 4, Room 214C. They may also be reached at 610-384-7711, ext. 5069.

To request a medical record, send the applicable forms (VA Form 10-5345, Request For and Authorization to Release Medical Records or VA Form 10-5345a, Individual's Request for a Copy of Their Own Health Information) to the following address:

The Release of Information Office must receive original, signed forms and they cannot accept forms via email. Requests for records may take approximately 10-14 days to process. There is no cost to send copies directly to a health care provider. If copies are for personal use, photocopying fees may be assessed.

You can also access your medical records through MyHealtheVet, an online health management tool. More information about MyHealthe can be found under Health Education.

Virtual Health Information Exchange (VHIE)

Do you see a Non-VA Health Care Provider? 'Connect Your Docs' by sharing certain parts of your Veteran health record between the Department of Veterans Affairs (VA), the Department of Defense (DoD), and selected private health care providers over a secure and confidential network known as the eHealth Exchange. Currently, VA and Lancaster General Health have the ability to share health records. This program is free and voluntary for Veterans.

How might connecting my docs benefit me?

Many Veterans receive a portion of their care from non-VA health care providers. Sharing certain parts of your Electronic Health Record (EHR) will help reduce the need for patients and families to carry records between health care providers as well as allow your non-VA health care provider to make more informed decisions about your care. In an emergency, immediate access to additional health care information could save your life.

How do I join?

Signing up is fast and easy. You may sign up in person, through the mail, by fax, or online.

NOTE: Do not email completed forms to the Release of Information office. They cannot be accepted via email.

What if I decide not to join?

Participation is entirely voluntary. Deciding not to participate will not affect your VA Health benefits or your relationship with your doctor and/or the Coatesville VA Medical Center.
