- Home Based Primary Care
- Home Hospice Care
- Skilled Home Care
- Respite Care
- Home Telehealth
- Home modifications
Veterans and caregivers with questions about the support available for Veterans to include Parkinson's, dementia, Alzheimer's disease and related disorders, ALS, diabetes, multiple sclerosis; post-traumatic stress disorder and schizophrenia are encouraged to attend.
Attendees will have the opportunity to talk with team members and receive practical information, advice and support about how the program can assist caregivers with the challenges they face every day. Attendees can also attend a question and answer session with the medical center executive leadership.
The Facility Leadership Engagement event will start at 1:30 p.m. and last for approximately 30 minutes to include the question and answer period. The Open House starts at 2:00 p.m. and will end at 4:00 p.m. at the Delaware County CBOC, located at 4883 West Chester Pike, Newtown Square, PA 19073.
